Partner with attending providers throughout hospitalization to promote effective and efficient utilization of clinical resources, ensuring quality, cost-effective care. Anticipate next steps and facilitate communication to maximize care efficiencies for the patient and family. Identify social determinants of health that increase risk for negative outcomes and coordinate post-acute plans with the ambulatory care management team. Address and resolve system problems impeding diagnostic or treatment progress while navigating value-based care and complex compliance, regulatory, and insurance requirements. Assess patients' and family's psychosocial risk factors and advocate for patient and family empowerment to make autonomous health care decisions. Ensure medical necessity, appropriate level of care, and timely implementation of plan of care in accordance with the hospital's Utilization Review Plan. Licensed to practice as a Registered Nurse in South Carolina with three years of acute care nursing experience. Day shift at Blount Memorial Hospital.
