Job Summary
Hospice Registered Nurse / RN Case Manager enables patients to spend quality time with loved ones and supports families by coordinating care at home. Build trusting relationships with patients and families, empower caregivers through education on at-home care, and collaborate with the care team to deliver comfort, peace, and dignity. Requires an active Ohio RN license, plus two years of nursing experience (hospice experience preferred), a valid driver’s license, and CPR/BCLS certification. Provides compassionate, patient- and family-centered hospice care in the home setting with opportunities for professional growth and a comprehensive benefits package.
