Job Summary
Hospice RN providing home-based care to patients with six months or less to live. Works within an interdisciplinary team to implement the patient’s plan of care, performing assessments, vital checks, breathing evaluation, pain management, and administration of prescribed medications and treatments. Educates families about prognosis, care plan changes, and hospice goals; ensures access to medical equipment and supplies; utilizesClinician Friendly EMR; engages holistically with patient and family to support comfort and quality of life. Requires active WI RN license, relevant nursing experience, CPR, and knowledge of home health regulations.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Wisconsin
- Minimum of 1 year nursing experience (ideally in hospice or home healthcare)
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing, strong communication skills, compassion
