Registered Nurse RN / Hospice Visit

Job Summary
Assess, treat, and communicate patient needs across cases within the Fayette, Shelby, Effingham, Jasper, and Richland counties. Conduct thorough patient and family assessments, contribute to individualized care plans, and administer medications per physician orders. Complete all visit types, identify nursing diagnoses, and record accurate clinical notes regarding pain and symptom management. Coordinate with community health partners, arrange necessary equipment, and communicate condition changes to physicians. Maintain clinical skills through continuing education and in-service participation. Requires an active RN license, graduate of an accredited nursing school, and clinical experience in hospice or home health. Schedule includes 8 AM–8 PM shifts with alternating 4-day and 3-day weeks including weekends.
Required Qualifications
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
- Valid driver's license
- Willingness to travel locally
- Flexibility and confidence working across varied patient assignments
- Strong assessment skills
- Sound clinical judgment under pressure
- Excellent documentation habits
- Communication skills
