Registered Nurse RN / Hospice Visit PRN

Assess, treat, and communicate patient needs with speed and clinical precision across varied assignments. Conduct thorough patient and family assessments to contribute to individualized care plans and identify nursing diagnoses from health assessment data. Administer medications and treatments per physician orders while recording accurate, timely clinical notes on pain and symptom management. Coordinate with community health partners to support seamless care transitions and ensure equipment and services are arranged. Maintain clinical skills through continuing education and in-service participation. Flexible weekend rotations available with day time hours.
