Job Summary
RN Case Manager in hospice coordinates care for patients in the Tri Cities (Saginaw, Midland, Bay City) and surrounding areas. Responsibilities include conducting comprehensive assessments, building and adjusting personalized care plans with physicians and care teams, administering medications per orders, maintaining accurate documentation, serving as the communication bridge between families, physicians, and community health partners, educating patients and caregivers, and participating in on-call rotation.
Required Qualifications
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing.
- One (1) + year Hospice, home health or related organization experience preferred.
