Registered Nurse RN / Hospice

Conduct comprehensive, ongoing assessments to understand each patient's evolving needs and administer medications and treatments per physician orders. Build and adjust personalized care plans in collaboration with physicians and care teams while maintaining accurate, timely clinical documentation. Serve as the communication bridge between families, physicians, and community health partners by educating patients and caregivers. Participate in on-call rotation per team policy within the Douglas, Coles coverage territory. Work 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends. As an RN Case Manager, you will be the clinical anchor for your patients and families, ensuring compassionate, unhurried end-of-life care delivered at home.
