Job Summary
Conduct comprehensive, ongoing assessments to understand each patient's evolving needs and build personalized care plans in collaboration with physicians. Administer medications and treatments per physician orders while maintaining accurate clinical documentation. Serve as the communication bridge between families, physicians, and community health partners by educating patients and caregivers. Participate in on-call rotation per team policy. Work 8 AM–8 PM in alternating 4-day and 3-day weeks including weekends in rotation across Bloomington, Pontiac, and surrounding areas.
Required Qualifications
- Active RN license in the state of employment
- Graduate of an accredited school of Nursing
- Compassion for patients and families navigating end-of-life care
- Strong clinical judgment and the ability to work independently in the field
- Excellent communication skills across care teams and families
- Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation
Desired Qualifications
- One (1) + year Hospice, home health or related organization experience preferred
