Registered Nurse (RN) Hospice

Hospice Registered Nurse (RN) responsible for planning, organizing, and directing home care services while reporting to the Clinical Supervisor. The role emphasizes community health education and tailoring services to patients and families in their homes and communities. Key duties include conducting comprehensive home assessments, developing individualized care plans with the interdisciplinary team, providing skilled nursing interventions (wound care, catheter care, infusion therapy, palliative care), educating patients and families on disease management and end-of-life care, monitoring and adjusting care plans, coordinating care transitions, offering emotional support and bereavement support, collaborating with community resources to address social determinants of health, and ensuring accurate documentation in electronic medical records. The position requires staying current with guidelines and participating in team meetings, quality improvement initiatives, and regulatory/compliance activities. The role also includes mentoring less experienced staff and maintaining professional standards in line with HIPAA and Medicare conditions of participation; travel to service areas in Las Cruces, NM and El Paso, TX (home-based hospice care).
