Job Summary
Conduct comprehensive patient and family assessments, develop individualized comfort-focused care plans, and deliver skilled nursing care prioritizing pain and symptom management. Act as the care coordinator by collaborating with physicians, social workers, chaplains, and aides to manage caseloads and adjust plans based on monitored conditions. Provide education, emotional support, and guidance to families navigating complex decisions while maintaining accurate documentation and participating in interdisciplinary team meetings. This field-based role requires an active RN license, one year of clinical experience, and experience with Homecare Homebase, offering medical, dental, vision, and 401(k) match benefits.
Required Qualifications
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Current CPR certification
- Valid driver's license and reliable transportation
