Job Summary
Conduct comprehensive patient and family assessments, develop individualized comfort-focused care plans, and deliver skilled nursing care with a focus on pain and symptom management. Act as the primary case manager by coordinating care across the interdisciplinary team, collaborating with physicians, social workers, chaplains, and aides to monitor conditions and adjust plans. Provide education, emotional support, and guidance to families navigating complex decisions while maintaining accurate, timely documentation and participating in IDT meetings. This field-based role requires an active RN license, one year of clinical experience, and experience with Homecare Homebase (HCHB). Candidates must possess strong clinical judgment, valid driver's license, and reliable transportation. Required Qualifications
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Current CPR certification
- Valid driver's license and reliable transportation
