Job Summary
Conduct comprehensive patient and family assessments, develop individualized comfort-focused care plans, and deliver skilled nursing care for pain and symptom management. Act as the primary care coordinator by collaborating with physicians, social workers, chaplains, and aides to monitor patient conditions and adjust care plans. Provide education, emotional support, and guidance to help families navigate complex decisions while maintaining accurate, timely documentation and participating in interdisciplinary team meetings. Requires an active RN license, minimum one year of clinical experience, and experience with Homecare Homebase. Field-based care in homes and facilities with high autonomy and strong team support.
Required Qualifications
- Active RN license (or compact eligibility)
- Graduate of an accredited nursing program
- Minimum 1 year of clinical nursing experience
- Strong clinical judgment and compassionate care approach
- Ability to manage a caseload independently
- Excellent communication skills in sensitive situations
- Basic EMR proficiency
- Current CPR certification
- Valid driver's license and reliable transportation

- Ability to quickly build trust with patients and families
- Experience with Homecare Homebase (HCHB)
