Job Summary
Conduct individualized presentations of hospice services to patients and families, assessing physical, psychosocial, environmental, safety, emotional, and spiritual needs while documenting findings in the EMR. Initiate professional IDT plans of care, articulate clinical courses for cancer, lung, heart, liver, dementia, and neurologic diseases, and manage symptom etiologies including nausea, dyspnea, and delirium. Collaborate with interdisciplinary teams, physicians, and facility staff to determine start of care orders, execute patient teaching, and maintain positive rapport with clinical and support staff. Work rotating 10-hour shifts from 9am to 7pm on weekends and holidays.
Required Qualifications
- Graduate of an accredited school of nursing
- Current Registered Nurse licensure in NY
- Valid Driver's License, free of serious violations
- Vehicle required for use when on duty or on-call
- 2 years of nursing practice as a professional Registered Nurse
