Job Summary
Assess patient physiological, psychosocial, and spiritual needs to develop individualized nursing and CNA plans of care. Perform thorough pain and symptom assessments, document findings clearly, and provide education to patients and families regarding end-of-life issues. Monitor patient status, evaluate treatment response, and initiate bereavement follow-up, including attendance at funeral services and memorial events. Coordinate care with physicians, interdisciplinary team members, and other agencies while overseeing CNA interventions. Participate in IDG care meetings, quality data reviews, and staff orientation for students and volunteers.
Required Qualifications
- Associate degree in Nursing
- One (1) year experience as a registered nurse
- Experience in hospice or other home care services
- Current RN compact/multistate license recognized by the NCSBN Compact State or licensed to practice as an RN in the state the team member is working
- Valid driver's license
- Acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart
- Proof of auto insurance
- Critical thinking skills
- Problem solving skills
- Knowledge concerning the process of grief/loss process and ability to demonstrate appropriate supportive interventions
- Familiarity with department policies and procedures and ability to demonstrate compliance with hospice standards of care
- Communication skills
- Ability to use listening skills that indicate understanding and promote accurate interpretation of others' concerns, motivation and feelings
