Job Summary
Assess patient physiological, psychosocial, and spiritual needs to develop individualized nursing and CNA plans of care, utilizing symptom management guidelines and standing orders. Provide comprehensive end-of-life care, including pain assessment, medication administration, and education for patients and families regarding grief and loss. Monitor patient status, update care plans based on changing needs, and coordinate with the interdisciplinary team, physicians, and CNAs to ensure continuity. Document all assessments, interventions, and plan changes accurately in medical records and participate in IDG care meetings and quality data reviews. Initiate bereavement follow-ups and attend funeral services as requested.
Required Qualifications
- Associate degree in Nursing
- One (1) year experience as a registered nurse
- RN - REGISTERED NURSE
- A valid driver's license
- An acceptable motor vehicle record, as defined by the Acceptable Motor Vehicle Record (MVR) Chart
- Proof of auto insurance
- Critical thinking skills
- Problem solving skills
- Knowledge concerning the process of grief/loss process and able to demonstrates appropriate supportive interventions
- Familiar with dept. policies and procedures and able demonstrates compliance with hospice standards of care
- Communication skills
- Ability to uses listening skills that indicate understanding and promotes accurate interpretation of others' concerns, motivation and feelings
- Day (United States of America) work shift
- Location: Blount Memorial Hospital
Desired Qualifications
- Experience in hospice or other home care services
- BSN preferred
- Successful completion of the Prisma Health Nurse Residency Program
- Successful completion of a comparable residency from another organization
