Job Summary

Assess hospice patients using Nursing Assessment and LCD criteria to identify physical, psycho-social, and environmental needs. Develop and document the plan of care while administering nursing care in patient residences using infection control measures. Coordinate care with the Interdisciplinary Group, patient families, and referring agencies, and communicate with attending and hospice physicians. Submit all required paperwork timely and participate in hospice-sponsored in-service programs and the QAPI program. Perform on-call responsibilities and provide services to patients and families as assigned. Requires two years of clinical nursing experience with one year in hospice, a current state RN license, and a valid driver's license. Involves prolonged standing, lifting up to 50lbs, and working flexible schedules with local travel.

Required Qualifications

  • Two (2) years experience as a Registered Nurse in a clinical care setting
  • one (1) year full time hospice
  • Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Current State license as a Registered Nurse
  • Current driver's license
  • Reliable transportation
  • valid auto liability insurance
  • ability to work a flexible schedule
  • ability to travel locally
  • some exposure to unpleasant weather
  • PRN emergency calls
  • Prolonged standing and walking
  • ability to lift up to 50lbs
  • ability to move patients
  • Ability to hear the nature of sounds

  • Ability to view tablet, Electronic Health Record, and review, assess, record or type data quickly and accurately
  • Ability to determine resources needed to provide quality patient care
  • Ability to travel to patient homes and office/support center locations as needed to deliver care or for education purposes
