Job Summary
Conduct home visits to clients in the Springfield and Claremont areas, administering medications, wound care, and comfort care while monitoring conditions and documenting doctor's orders. Report clinical changes to the Clinical or Client Services Manager and perform necessary client assessments. Maintain current RN licensure in VT & NH with one year of recent clinical experience, with prior hospice or home care experience preferred. Work full-time with occasional on-call after hours and weekends.
Required Qualifications
- A current license as a Registered Nurse (RN) in VT & NH
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Ability to travel to cases as assigned
- Occasional on call after hours and/or weekends
Desired Qualifications
- Prior home care and/or hospice experience
- Solid computer skills
- Prior experience with electronic medical records (EMR)
