Job Summary
Make home visits to clients in the Springfield and Claremont areas, administering medications, performing wound care and treatments, and providing comfort care. Monitor clients' conditions, report changes to the Clinical or Client Services Manager, and document doctors' orders while conducting necessary assessments. Work full-time with occasional on-call availability after hours and weekends. A current VT & NH RN license and one year of recent clinical experience are required, with prior hospice or home care experience preferred.
Required Qualifications
- A current license as a Registered Nurse (RN) in VT & NH
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Ability to travel to cases as assigned
- Occasional on call after hours and/or weekends
Desired Qualifications
- Prior home care and/or hospice experience
- Solid computer skills
- Prior experience with electronic medical records (EMR)
