Job Summary
Conduct routine and emergency assessments, provide clinical and psycho-social services, and manage case plans for patients and families in home hospice settings. Document observations, treatments, and plan of care changes at the bedside, then submit narratives, time logs, and paperwork to contracted facilities by 10 a.m. the following day. Communicate with the interdisciplinary team, medical director, and attending physicians to alleviate symptoms and prevent unwarranted hospitalizations, while offering after-hours emergency support as part of on-call responsibilities. Maintain maximum comfort and coping for patients during crisis periods through proactive and reactive interventions.
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Active and unencumbered Registered Nurse license in state(s) of employment
- Current CPR certification
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals
- Ability to compute rate, ratio, and percentage
