Job Summary
Conduct routine and emergency assessments for hospice patients and families, providing clinical and psycho-social services through case management. Collaborate with the interdisciplinary team to coordinate care, update plans of care, and prevent unwarranted hospitalizations by addressing symptom management and crisis needs. Document observations, treatments, and interventions at the bedside, submitting narratives and time logs by 10 a.m. daily. Provide physical and emotional support during periods of crisis, educate caregivers on disease processes and wound care, and communicate patient needs to physicians and medical directors. Maintain on-call availability for after-hours emergency support while flexing schedules to meet agency staffing requirements.
Required Qualifications
- Associate or Bachelor's degree in Nursing
- Active and unencumbered Registered Nurse license in state(s) of employment
- Current CPR certification
- Ability to lift and move objects up to 50 pounds
- Close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus
- Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations
- Ability to write reports, business correspondence, and procedure manuals
- Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties
- Strong written and verbal communications
- Ability to understand, read, write, and speak English
- Articulates and embraces integrated healthcare at home philosophy
