Job Summary
Provide care and services for patients residing in private homes, skilled nursing facilities, and assisted living facilities as a contributing member of the interdisciplinary team. Conduct psych-social assessments, interventions, and coordinate with nurses, social workers, chaplains, volunteers, and CNA leadership. Manage visits, documentation, and meetings while prioritizing urgent needs through phone, email, and written communication. Work independently using an electronic medical records system and adhere to hospice rules and regulations. Requires current RN licensure, reliable transportation, and valid driver's license.
Required Qualifications
- Current Registered Nurse (RN) licensure (in good standing) in the state(s) of practice
- Valid driver's license
- Auto insurance
- Reliable transportation
- Ability to learn an electronic medical records system
