Registered Nurse RN / Hospice After Hours

Assess, treat, and communicate patient needs across a coverage territory including Bloomington, Pontiac, and surrounding areas during overnight shifts from 8 PM to 8 AM. Conduct thorough patient and family assessments to contribute to individualized care plans while administering medications per physician orders. Complete all visit types required to meet overnight patient and family needs, ensuring accurate clinical documentation of pain and symptom management outcomes. Coordinate with community health partners to support continuity of care and maintain necessary equipment. Maintain clinical skills through continuing education and in-service participation while providing a calm, skilled presence to families during distress.
