Registered Nurse (RN) | Hospice After Hours

Respond to after-hours patient needs in a patient's home as directed by the attending physician, providing skilled nursing care and administering medication to prioritize visits for high-risk patients and decrease re-hospitalizations. Monitor patient conditions, perform OASIS assessments, and develop individualized care plans that adjust as needs change. Oversee and supervise total care provided by nurse aides and LPNs while collaborating with the interdisciplinary team to ensure continuity of care during admissions, transfers, and discharges. Educate patients and families on disease processes using teach-back methods and complete all clinical documentation following agency protocol and Medicare guidelines. If minimal needs occur during a shift, manage time autonomously to assist with chart reviews or patient phone calls from your home. This role supports a team focused on compassionate quality care and producing positive outcomes for the patient population.
