Registered Nurse (RN), Hospice Admissions

Complete hospice admissions and skilled nursing visits while building meaningful connections with patients and families. Provide education on conditions such as CHF, COPD, and Diabetes, and perform wound care and medication management including changing dressings and medical devices. Accurately document patient conditions and communicate changes to physicians, partnering closely with interdisciplinary hospice teams to ensure seamless care transitions. This role requires hospice experience, an active Missouri or Compact RN license, and the ability to work independently while collaborating with intake coordinators, consultants, and medical directors. Full-time 40-hour shifts run Monday through Friday.
