Job Summary
Provide home-based hospice care to patients with six months or less to live; collaborate with an interdisciplinary team to carry out the patient’s plan of care; perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medications and treatments; ensure accessibility of medical equipment and supplies; educate family on prognosis, changes in care plan, medication and hospice goals; take a holistic approach to care that engages the patient’s mind, body, spirit and family.
Required Qualifications
- Graduate of an accredited nursing program
- Active RN license in Wisconsin
- CPR certification
- Knowledge of state and federal home health regulations
- Clinical proficiency in nursing
- Strong communication skills and compassion
- Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
