Job Summary
Conduct home hospice visits for clients in designated geographic territories while following designated care plans. Administer medications, perform wound care and treatments, and monitor client conditions by reporting changes to the Clinical or Client Services Manager. Execute and document doctors' orders, perform client assessments, and manage case coordination using a state-of-the-art touch pad tablet. Maintain accurate records of observations, interventions, and evaluations. Requires a current SC RN license, one year of recent clinical experience, and ability to travel to assigned cases.
Required Qualifications
- A current license as a Registered Nurse in SC
- A minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma
- Ability to work independently and manage time effectively
- Strong interpersonal skills
- Solid computer skills
- Ability to travel to cases as assigned
Desired Qualifications
- Prior home care experience
- Prior experience with electronic medical records (EMR)
