Job Summary
Conduct home health visits to administer services such as IVs, wound care, and assessments while maintaining patient independence. Perform comprehensive patient assessments, document findings in Synergy Software, and obtain physician orders. Collaborate with the Clinical Team Manager to initiate, re-evaluate, and revise care plans, coordinating with physicians and other team members for significant changes. Supervise home health aides per regulatory requirements and manage transfers, resumption of care, and discharges. Demonstrate knowledge of OASIS documentation and state regulations while traveling to patients' homes. Requires current CA RN license, BLS certification, IV/phlebotomy skills, and ability to work weekends and holidays.
Required Qualifications
- Visiting RN for Home Health: 1 year
- Nursing: 1 year
- Current CA RN License
- BLS from the American Heart Association and/or the American Red Cross
- NSO Nursing License Insurance
- IV/Phlebotomy Certified
- Graduate of National League for Nursing accredited school of nursing
- Maintains a current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order
- Able to lift, position, or transfer patients
- Able to lift supplies and equipment
- Visual acuity and hearing to perform required nursing skills
- One weekend a month
- Two holidays per year (one winter and one summer)
Desired Qualifications
- Comprehensive knowledge of OASIS documentation
- Minimum of two years experience, at least one of which is in the area of public health or home care nursing
- Self directed and able to work with minimal supervision
- Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist
- Shows ability to organize and prioritize workload independently
- Management experience
