Job Summary
Perform skilled nursing services and coordinate home health care for assigned patients, including medication administration, wound care, and therapeutic treatments. Conduct nursing assessments, develop plans of care in conjunction with physicians, and re-evaluate patient needs using the OASIS data set. Implement health teaching, counseling, and preventive procedures while supervising LVNs and home health aides. Document all communications, care conferences, and discharge planning, ensuring compliance with all applicable laws and policies. This field-based role requires traveling around San Antonio and surrounding counties with on-call rotations for weekdays and weekends.
Required Qualifications
- Graduate of an accredited school of nursing
- Current unrestricted Texas RN license or compact license
- BLS certification
- IV Certification
- 1+ years of experience in home health care or related environment
- Wound Care
- Developed computer skills
- Ability to move and transfer patients
- Access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- Tuberculosis screening
- Proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
- Required on calls (weekday and weekends)
- Mileage reimbursement
- Shift differential
