Job Summary
Perform nursing assessments, develop and evaluate plans of care, and provide skilled services including medication administration, wound care, and therapeutic treatments. Re-evaluate patient needs using the OASIS data set, implement health teaching and counseling, and coordinate services with the Clinical Team Manager to recommend additional care within 24 hours of start. Assess conditions during every visit, communicate findings daily, and manage care duties such as notifying physicians, supervising LVNs, attending case conferences, and documenting discharges. Work Friday through Monday shifts in a field-based role traveling around San Antonio and surrounding counties. Requires a Texas RN license, BLS and IV certification, wound care experience, and a driver's license.
Required Qualifications
- Graduate of an accredited school of nursing
- Current unrestricted Texas RN license or compact license
- BLS certification
- IV Certification
- 1+ years of experience in home health care or related environment
- Wound care experience
- Proven developed computer skills
- Ability to move and transfer patients
- Driver's License and access to reliable transportation that will enable you to travel to client and/or patient sites within a designated area
- Tuberculosis screening
- Proof of immunity to Measles, Mumps, Rubella, Varicella, Tetanus, Diphtheria, and Pertussis through lab confirmation of immunity, documented evidence of vaccination, or a doctor's diagnosis of disease
- Friday - Monday shifts
- Must be able to pass a drug test
