Registered Nurse, RN - Home Health - PRN - Weekend

Conduct comprehensive in-home assessments, develop individualized care plans, and deliver skilled nursing care including medication management and treatments. Serve as the case manager by coordinating care across therapists, aides, and physicians while monitoring patient progress and making clinical decisions. Educate patients and families to support disease management, coordinate discharge planning to reduce readmissions, and maintain accurate EMR documentation. Supervise Home Health Aides as needed and participate in on-call rotation. This field-based role offers high clinical autonomy within a collaborative team environment focused on patient recovery and long-term independence at home.
