Registered Nurse, RN - Home Health - PRN

Conduct comprehensive in-home assessments, develop individualized care plans, and deliver skilled nursing care including medication management and interventions. Serve as the clinical lead and case manager by coordinating care across disciplines, monitoring patient progress, and driving outcomes from admission through discharge. Educate patients and families, coordinate care transitions to reduce readmissions, maintain accurate EMR documentation, and supervise Home Health Aides. Participate in on-call rotation as appropriate. This field-based role requires an active RN license, one year of clinical experience, and proficiency with EMR systems. You will practice with high autonomy within a collaborative team focused on reducing hospitalizations and supporting long-term independence at home.
