Registered Nurse RN Home Health (PRN)

Job Summary
Perform comprehensive patient assessments, including OASIS assessments at required intervals. Develop, initiate, and revise individualized plans of care as patient needs evolve. Deliver skilled nursing services and treatments as outlined in the plan of care. Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies. Collaborate with the interdisciplinary team to ensure seamless, holistic care. Complete 10+ home health visits per week with weekend and holiday on-call rotations. Work with patients in north Hillsborough county to help them recover and maintain independence at home as part of a mission-driven, not-for-profit organization dedicated to Full Life Care for All.
Required Qualifications
- Graduate of an approved nursing program
- Active Florida RN license
- Minimum of 1 year of direct patient care experience
- Willingness to participate in weekend and holiday on-call rotations
Desired Qualifications
- BSN or MSN degree
- Prior home health RN experience
