Job Summary
Support patient recovery and ability to stay at home by educating families on disease management and providing clinical oversight to the interdisciplinary team. Case manage care for patients with advanced illnesses in partnership with health systems and long-term care partners. This PRN position provides additional coverage where needed for a home health registered nurse. Requires an RN license, two years of nursing experience, home health experience preferred, current driver's license, and CPR/BCLS certification.
