Registered Nurse RN Home Health PRN

Registered Nurse Home Health PRN provides in-home nursing care to patients across assigned territory. Responsibilities include initial evaluation and ongoing assessment of physical, psychosocial, and environmental needs; planning, coordinating, implementing, and evaluating the plan of care; documenting findings in clinical records and Interdisciplinary Team reports; communicating significant changes to the Clinical Supervisor, physician, facility, and other team members; and revising the plan of care as needed. Requires current RN license, 1+ year clinical experience, ability to lift up to 50 pounds, and willingness to travel up to 50% within service area. Must have a dependable vehicle, valid driver’s license, and auto insurance.
