Job Summary
Registered Nurse Home Health at Elara Caring provides at-home nursing care to patients in their homes, performing initial evaluations and ongoing assessments, developing and implementing plans of care, coordinating with an Interdisciplinary Team, and communicating significant health changes to the Clinical Supervisor, physician, and care team. The role requires uplifting patient privacy per HIPAA, documenting care across clinical records and reports, and traveling within the branch/office coverage area to perform visits and follow-ups while ensuring high-quality care and compliance with organizational standards.
Required Qualifications
- Current, unrestricted state RN licensee
- 1+ year experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws
- Associates Degree or Bachelor’s Degree in Nursing preferred
- Previous Home Health or Hospice Experience preferred
- Previous experience with HomeCare HomeBase preferred
