Registered Nurse RN Home Health PRN

Assess assigned case loads of home health care patients to identify physical, psychosocial, and environmental needs through documentation, clinical records, and interdisciplinary team reports. Plan, coordinate, implement, and evaluate treatment plans while making initial and follow-up visits according to patient and facility requirements. Communicate significant findings, health condition changes, or unsafe facility conditions to the Clinical Supervisor, physician, and other personnel involved in patient care. Revisions to the plan of care must be made in response to identified issues and notified to relevant team members. Maintain patient and staff privacy in accordance with HIPAA regulations. Requires current, unrestricted state RN licensure, at least one year of clinical care experience, and ability to travel 50% within the coverage area with a dependable vehicle.
