Job Summary
Registered Nurse Home Health provides in-home nursing care across patients' living environments. The role includes assessing patients and families to identify physical, psychosocial, and environmental needs, planning, coordinating, implementing, and evaluating the plan of care, making initial evaluations and follow-up visits, communicating significant findings to the Clinical Supervisor/physician/facility, and revising the plan of care as needed. Travel is required within the branch/coverage area to deliver care and ensure HIPAA-compliant privacy and documentation. Requires current RN license, typically 1+ year of clinical nursing experience, ability to lift up to 50 pounds, and reliable vehicle with valid license and insurance; Home Health/HomeCare HomeBase experience and advanced degrees are preferred.
Required Qualifications
- Current, unrestricted state RN license
- 1+ year experience in a clinical care setting as a nurse
- Ability to frequently lift, push, pull, and support up to 50 pounds
- Able and willing to travel within branch/office coverage area
- Must have a dependable vehicle, valid driver’s license, and current auto insurance
- Associates Degree or Bachelor’s Degree in Nursing is preferred
- Previous Home Health or Hospice Experience is preferred
- Previous experience with HomeCare HomeBase is preferred
