Registered Nurse RN Home Health PRN

Assess assigned home health care patients and families to identify physical, psychosocial, and environmental needs, then make initial evaluations and re-evaluations during each visit. Plan, coordinate, implement, and evaluate the patient's treatment plan while revising it in response to identified care issues. Communicate significant findings, health condition changes, or unsafe facility conditions to the Clinical Supervisor, physician, and other personnel involved. Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule. Ensure all activities align with the vision of Elara Caring's board and leadership. Requires current, unrestricted state RN license with one year of clinical experience; ability to lift up to 50 pounds; and willingness to travel 50% with a dependable vehicle. Compensation ranges from $40.00 to $45.00 per point annually, with additional earnings for on-call work.
