Job Summary
Conduct comprehensive assessments of patients' health status, including physical, emotional, and social factors. Develop and implement individualized care plans tailored to each patient's needs and goals. Provide patient education and support to promote self-management, while administering medications and treatments as ordered. Coordinate care with physicians, therapists, and social workers to ensure continuity, and document all activities to communicate progress to the healthcare team. Participate in quality improvement initiatives and maintain professional knowledge through continuing education. Adhere to all applicable laws, regulations, and ethical standards of nursing practice.
Required Qualifications
- Current and unrestricted RN license in the state of Florida
- Strong clinical skills and ability to perform evidenced based nursing practices
- 1-3 years of clinical experience in a healthcare setting, preferably in home health, hospice, or community health
- Excellent communication and interpersonal skills with the ability to work effectively with patients, families, and healthcare providers
- Physical Exam and Two-Step TB Test or Chest X-Ray (no older than 6 months)
- Yearly Mandatory in-service
- Proficient in using electronic health records and other healthcare technologies
- Valid FL driver's license and reliable transportation (Auto Insurance must also be presented)
- CPR certification required (Original Card)
- Level 2 Criminal Background Check
