Job Summary

Assess patients' physical and psychosocial needs, establish treatment plans, and manage assigned caseloads within the team model of care delivery. Complete physician orders, OASIS documentation, and medication reconciliation while promptly triaging visits, messages, and phone calls according to priority. Collaborate with patients, families, and multidisciplinary providers on care planning and discharge coordination, adapting to new situations without compromising performance. Maintain expert knowledge of third-party payers and regulatory bodies to ensure safe, efficient home health services that avoid hospitalization. Full-time days shift with weekend/holiday commitment required.

Required Qualifications

  • Massachusetts/New Hampshire RN Licensure
  • Valid state issued Driver's License
  • One (1) year of acute medical/surgical nursing experience
  • Cardiopulmonary Resuscitation (CPR) Certification
  • Weekend/Holiday commitment
  • Ability to utilize Security when and if needed for any potential unsafe situations
  • Ability to adapt to new and unusual situations without affecting work performance negatively
  • Ability to demonstrate excellent physical assessment and care planning skills
  • Current knowledge of pharmacology and medication administration and reconciliation
  • Ability to demonstrate ability to cope with patient/family emotional stress and provide appropriate supportive care
  • Ability to effectively manage assigned caseload, within the team model of care delivery
  • Ability to establish a daily work plan based upon patient/family priorities of service and total area needs
  • Ability to promptly triage patient visits, messages, and phone calls according to priority and urgency
  • Ability to coordinate care and discharge planning with other team members during case conferences
  • Expert working knowledge of all third part payers and regulatory bodies and agency policies on issues related to documentation and care provided
  • Ability to communicate and collaborate with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions needed
