Registered Nurse / RN (Home Health)

Conduct thorough assessments of patients and families to determine home care needs, providing comprehensive physical assessments and documenting illness histories. Develop and update individualized care plans with active participation from patients and families, while identifying discharge planning needs to ensure they are addressed before patient discharge. Offer counseling to patients and families to address nursing and related health needs. Requires current and valid state RN license, minimum of one year of recent acute care experience, and Basic Life Support certification. Valid driver's license and auto insurance required. Pay range $55-$60/hour DOE; schedule Mon-Fri 8AM-5PM in Salem and surrounding locations.
