Job Summary
Provide nursing care for clients in patients' homes, coordinating with an interdisciplinary team to assess physical, psychosocial, and environmental needs (including OASIS), develop and revise the Plan of Care, administer nursing procedures, supervise other nursing personnel, participate in case conferences, maintain timely documentation, and provide on-call services as needed. Must have active RN licensure and be able to travel locally; role includes infection control, OSHA compliance, and delivering high-quality home health services in Graham, TX (serving TX, FL, NV, GA).
Required Qualifications
- Current RN license in the United States
- Graduation from an accredited Registered Nursing Program
- Current CPR certification
- Reliable transportation and valid driver’s license
- Ability to read, write, and comprehend English
- Preferred: BSN
- Current licensure as RN in applicable states where practice occurs
- Ability to lift up to 50 lbs and perform home health duties
- Proven nursing skills and interpersonal communication
