Registered Nurse (RN) - Home Health

Provide nursing care for home health clients in their residences, coordinate care with an interdisciplinary team and families, and perform ongoing assessments (including OASIS). Regularly reevaluate nursing needs, develop/initiate/revise the Plan of Care to ensure quality and continuity of care, and initiate appropriate preventive and rehabilitative nursing procedures. Manage discharge planning, ensure effective communication with clients, families, and health professionals, participate in case conferences, apply infection control and universal precautions, coordinate changes in patient condition with physicians, supervise other nursing personnel (e.g., LVN/LPN, HHA, CNA), and perform on-call responsibilities. Maintain compliance through timely documentation and ongoing professional development, with opportunities for career growth and continuing education. Requires strong nursing skills, interpersonal communication, decision-making, and the ability to travel locally to patient homes.
