Job Summary
Provide home-based care to patients with illnesses, injuries, and chronic diseases, often following hospital or rehab discharge. Examine patients, conduct assessments, document progress, and report changes to physicians. Educate patients on their care plans, medications, therapy, diet, and exercise. Coach family members on patient plans, medication management, and home safety. Coordinate discharge goals and work with a team of physicians, LPNs, CNAs, and aides. Part-time role with a minimum expectation of 5 visits or 10 hours per week. Compensation ranges from $50-$150 per visit plus mileage and travel reimbursement. Requires an active Colorado RN license, 1200 hours of RN experience, and BLS certification.
Required Qualifications
- Active Registered Nurse (RN) License in Colorado
- Graduate of an accredited school of nursing
- At least 1200 hours of experience as an RN
- Health care CPR/BLS Certified (demonstration course required)
- Pass a background check and drug test (we are required to test for THC)
Desired Qualifications
- Home health care experience preferred
