Registered Nurse (RN) - Home Health

Conduct home health nursing care including assessment, medication management, documentation, and supervision of other nursing personnel. Coordinate the implementation of patient care plans, advocate for patient choice, and provide education and counseling to patients and families. Maintain effective communication with individuals, practice infection control, and respond to patient and family calls by phone. Participate in quality assurance, performance improvement, and in-service training while leading the interdisciplinary team. Complete all documentation within policy timeframes and maintain confidentiality. Work Monday through Friday, 8:00 AM to 4:30 PM, with on-call services as assigned.
