Job Summary
Per diem Registered Nurse (RN) – Home Health in Hatfield, PA. Conduct comprehensive initial assessments and develop, implement, and continuously evolve patient-centered home care plans. Perform ongoing nursing assessments and adjust care plans in real time based on changing conditions. Administer treatments, medications, and interventions as prescribed; provide health education and hands-on guidance to patients and families. Support proactive discharge planning to ensure seamless transitions. Serve as Case Manager when designated, coordinating holistic care and ensuring continuity. Maintain clear, timely, and accurate clinical documentation. Collaborate with physicians, care coordinators, and interdisciplinary teams; act as liaison between patient, family, and healthcare providers. Participate in case conferences and advocate for optimal patient outcomes. Participate in on-call rotation for 24/7 care continuity. Coordinate equipment, services, and resources for effective home treatment. Supervise and evaluate home health aides every 14 days to ensure quality care delivery.
Required Qualifications
- Graduate of an accredited nursing program (Associate or BSN preferred)
- Active RN license in the state
- Current CPR certification
- Valid driver's license
- 1–2 years of acute care experience
- Home health care experience preferred
