Registered Nurse (RN) - Home Health - Full Time

As a Home Health RN at VitalCaring, you will lead clinical care for patients in their homes, owning the plan of care from admission through discharge. You will perform in-home assessments, develop and manage individualized care plans, deliver skilled nursing interventions and medications, serve as the case manager coordinating across therapists, aides, and physicians, monitor patient progress, educate patients and families on disease management, coordinate transitions and discharge planning to reduce readmissions, maintain EMR documentation, supervise Home Health Aides, and participate in on-call coverage. You’ll work with strong clinical autonomy in a field-based role, delivering high-impact care and building meaningful patient relationships within an assigned territory. A valid RN license (or compact eligibility) and CPR certification are required, with at least one year of RN experience; additional experience in home health, post-acute care, or case management and familiarity with Homecare Homebase (HCHB) are advantageous.
