Registered Nurse RN, Home Health - Full Time, Part Time

Assess, plan, implement, and evaluate patient care using the nursing process, including physical, social, and psychological status. Collaboratively develop individualized care plans with patients, families, and the interdisciplinary team while directing care delivery through delegation and resource management. Evaluate patient responses to interventions, revise plans as needed, and ensure accurate, timely documentation within legal and ethical standards. Maintain BLS certification and manage patient transfers, equipment, and supplies in a busy clinical environment. Requires current RN licensure, BSN within four years for new hires, and experience with complex patient needs.
