Job Summary

Collaborate with the care team to develop and implement a Plan of Care, maintain regular communications with clinical management, and ensure timely, accurate documentation. Requires current RN license, Home Health/OASIS experience, and CPR certification. Required Qualifications
- Graduate of an accredited Registered Nurse educational program
- Current Registered Nurse (RN) license within the state of practice
- Home Health/Oasis experience
- Current CPR certification
- Excellent verbal, written, organizational, collaboration, multi-tasking and time-management skills
- Strong initiative, follow through, process orientation, attention-to-detail
- Reliable transportation
- Valid driver's license
- Current automobile insurance
