Job Summary
Conduct one-on-one patient care and patient/family teaching in the home setting. Perform patient assessments and collaborate with the care team to develop and implement a Plan of Care. Maintain regular communications with the clinical management team and ensure timely, accurate documentation. Requires clinical home health and OASIS experience with visit rates between $50 and $135.
